For patients with a"chubby" face, sometimes the filler will add a little too much bulk if you try to correct the area fully, and surgery may be the answer. Turning up the corners of the mouth can be achieved by using a combination of fillers and Botox. A small amount of Botox can be injected into the depressor anguli oris, the muscle that turns down the mouth corners. In about a week, the downward pull on the corners relaxes and the upward pull of the smile muscles takes over and essentially turns your frown "upside down!." Fillers like Restylane, Juvederm and Radiesse also can be used to help turn up the corners of the mouth and fill in the pre-jowl areas and marionette lines.

This is a temporary fix that will last several months.A relatively simple surgery under local anesthesia that I have used is a corner of the mouth lift which hides the incisions on the lip border. This gives a much longer lasting result.
There can be many reasons for the sagging at the corner of the mouth. Because of this it is one of the more interesting areas of the face. Babies often will have this. It can be due to a relative excess of fat. It can also be caused by loss of soft tissue in the groove (marionette line). A recessed or hypoplastic mentum or chin bone may be a reason. Finally, aging itself can cause a decent of the mid-face and cheek soft tissue and a lifting in a direction perpendicular to the line can be helpful. A photo comparison with your face at a younger age will help with the diagnosis.
