I’m 6 weeks post op. I had fat grafted to my inner thighs to address irregularities from past lipo. Some have softened a bit while others are still hard and lumpy. I also developed a small marble size ball on my upper right inner thigh that appears to be fat necrosis. Will these harder lumpy areas soften and smoothen? I just really want a soft smooth contour. I want to feel like I have normal thighs again. Help!

Answer: The problem with correcting excess of liposuction with fat grafting The inherent problem or difficulty with the situation you presented yourself with to your last provider was your liposuction done initially removed all sources of host tissue needed to support grafted fat done six weeks ago. In order for a grafted fat to survive it needs to be surrounded by live vascular tissue. This was removed during your initial liposuction procedure you were hollowed out leaving you with skin over muscle. It's possible to graft fat directly into the muscle but that is not what you need. it's not possible to graft fat into the dermis of the skin or at least not very well. Now the grafted fat Has died and is slowly converting to scar tissue. When attempting to build up excessively removed fat from overly zealous liposuction it's generally better to do so with small amounts of fat in multiple session. At six weeks you have a reasonably good preview of what your final results will look like. Expect the firm areas to continue shrinking to some degree. Unfortunately poorly done liposuction especially it went too much fat has been removed is very difficult and at times not possible to correct. Sometimes direct skin excision's can be options but I would not recommend this to most people and certainly not without an examination first. I making assumptions and to get an accurate assessment and examination is always needed. Best of luck, Mats Hagstrom M.D.
