Why has my implant moved and what procedure to have next? (Photo)

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

Based purely upon your photos, it does not appear as though your right buttock implant is submuscular, (or intramuscular), more likely, it is subcutaneous on top of the muscle. Hence, why it is riding up so high upon your lower back and out of your buttock cheek/muscle compartment. It also appears as though your implants are round, and therefore do not extend the entire length of your gluteus muscles. If my suspicions are correct, the best solution is to remove these round implants, create a new 100% intramuscular pocket, then replace them with Stanton Anatomic®️ oval shaped buttock implants.

Please note that an extremely few number of board certified plastic surgeons actually specialize in buttock or hip implants, so you may need to travel far and away from home to find this particular surgeon. Glad to help.

The lower and outer limits of where the implant can be placed are clearly defined. Placing it lower is not possible due to the risk of nerve damage. If placed higher, it may result in an unnatural appearance. If the space created is large in relation to the implant size, the implant may move within the space. However, if the space is too high, closing it with sutures and increasing the size of the implant can help reduce the issue.

Hello! It is normal for the buttocks to look sagging and empty after an extraction, because they were stretched for a long time by the presence of the implants. Sagging is normal but it is nothing that cannot be corrected. You should consider that to make a more realistic decision you must first have at least one physical or virtual consultation to make a more accurate decision with an experienced surgeon.
