The other important issue if you have large breast is that you may have ptosis and it could be necessary to do something else and maybe periareolar incision wouldn't be the best option. The periareolar incision is a nice incision for breast augmentation because the scar hides well at an area where there is a natural change in color from the areola to the skin. However, this incision is associated with increased risk of capsular contracture because the breast ducts that you must go through are not sterile. Once the incision has been made, no matter if it is inframammary, periareolar or transaxillary, the implant pocket can be either subpectoral (behind muscle) or subglandular (in front of muscle). The decision for pocket placement would be dependent on your individual anatomy.
