I seen a cosmetic dermatologist close by me today for the first time. I asked him for suggestions regarding my face. He told me my forehead lines are etched into the skin and botox may gradually decrease the lines over a year, but may not do much either. He suggested dermapen? I have lines that run vertical by my mouth when I smile and also the bridge line from corner of nose to the mouth. He suggested 1 syringe of Vollure? Is Juvaderm the best? Is Vollure good? Any help is appreciated.

Answer: Microneedling for etched in lines on forehead Yes, it does appear you have etched in lines. I would recommend regular treatment with Botox or Dysport to ensure the lines do not worsen and skin resurfacing treatment like microneedling to help with the etched in lines. I prefer the SkinPen device for microneedling.

Answer: Treatment of Etched Lines The skin and the underlying facial muscles are in a constant contest for supremacy. As the skin thins and the muscles hypertrophy, the skin begins to accordion and wrinkle when the muscle pulls on it. Initially, the wrinkles produced go away when the muscle relaxes. Over time, however, the lines become etched. Botox can correct the lines before the etching begins. However, once the indelible lines are present, it takes more than just treatment with Botox. Also, there are areas like in the cheeks and near the mouth where you cannot use Botox. In these areas you need to thicken the dermis (the support layer of the skin). You can do this by use of Retin-A, use of things like the DermaPen or certain Fraxil like lasers that stimulate collagen production. I prefer Retin-A. You can also fill the lines with a filler like Juvederm Volbella or Vollure and support the skin with cross crossing the filler under the involved skin. This allows the skin to use some of the injected hyaluronic acid to build its own collagen and helps support the skin over time more than just from the injection. (There is now good data to support this function.)Without seeing the cheek and nasolabial area, I cannot comment on your other concern, but, yes, Juvederm Vollure is an excellent filler. However, the physician doing the injection is even more important.
