Benefits of diced cartilage fascia for rhinoplasty & does an oily nose mean thick skin?

What are the benefits/advantages of Diced cartilage fascia for Rhinoplasty. Which surgeons in NY, CA and ATL use this technique? And, how can I tell if I have thick skin which will cause limitations during rhinoplasty. I have a very oily nose. My dermatologist always said that I have thin skin when they treated me for rosacea. Does an oily nose mean thick skin?

In rhinoplasty, using the patients own cartilage or fascia is always preferable to using some other material, such as an implant. Cartilage grafting, including diced cartilage, and fascia grafting is routinely used by almost all experienced rhinoplasty surgeons on nearly all rhinoplasty cases, whether you're in NYC, CA or ATL. Thick skin is determined by examination and usually means 1. Less than perfect nasal tip definition before surgery and 2. Less than perfect nasal tip definition after surgery. Thick skin is almost always a limitation and should be discussed as such during a rhinoplasty consult. There are ways to thin the skin during rhinoplasty. Although, these techniques should be done only by the most experienced surgeons. Thin skin on the cheeks and face does not necessarily correlate to thickness of the skin of the nose. Even the skin on the nose varies in thickness between different areas. For example, the skin over the bone (bony dorsum) is almost always much thinner than the nasal tip skin. Experienced rhinoplasty surgeons will discuss this during the consultation as well. Oily skin usually correlates with thick skin. Although, this is a generalization, not a rule. We use this all the time. The diced cartilage helps to either provide fullness where we need it, or, more commonly, to lift the bridge of the nose.  Thick skin and oily skin don't always go together, though they can.  A picture will be very helpful!
