Nose is very round, fat, and bulbous - best way to proceed to get a more defined tip? (photos)

Hi and thanks for the question! What you are looking for is a tip rhinoplasty. Specifically this will help reshape and reposition your lower lateral cartilages. It would be difficult to get your nose to look exactly like the one in your 'what I would like' photo but I would expect you could see a very significant improvement to the tip bulbosity by having a tip rhinoplasty performed. It is difficult to see the rest of your nose to see if any work would be required on the other parts of your nose, but for the tip different options would be cephalic trims, cephalic turn-in flaps, lateral crural repositioning, and lateral crural extension grafts to help shape and reposition the cartilages in your nasal tip causing the bulbous appearance. Based on your photos, your nasal tip appears to deviate towards your right side, and there is also a C-shaped nasal appearance, and a bulbous tip appearance. These issues may be addressed with rhinoplasty surgery. Thank you for your question. There are several ways to make changes to the nasal tip during surgery. The amount of change desired should be discussed with your surgeon. The goal most likely will not be achievable since your skin is thicker, but changes to get your nose closer to the goal are possible. I hope this is helpful, and all the best to you.

Thank you for your question. I think rhinoplasty is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you.

There are simple ways to deal with the bulbous tip and there are complicated ones. Complicated methods will most likely involve open rhinoplasty with cartilage grafting. Go on multiple consults because no two surgeons will do exactly the same thing

Thank you for sharing your photos! It is difficult to say without examining you in-person, but there are different approaches to rhinoplasty. Rhinoplasty can be performed through open or closed techniques. One advantage of the closed rhinoplasty is there are no visible scars. I perform rhinoplasty as an outpatient procedure under IV sedation. A nose should fit your facial features, skin tone, ethnicity, and height. A good surgeon will work with you to arrive at the optimally harmonious look. I suggest making an appointment with a plastic surgeon for consultation to discuss your options.

The photograph to show bulbous nasal tip cartilages. You also appear to have thick skin in the tip of the nose which is going to restrict how much refinement you're going to see; therefore it's important to have realistic expectations. A closed rhinoplasty approach can give improvement with all the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. When patients have thick skin in the tip of the nose, it's very important for follow up appointments after the surgery to administer steroid shots in the nasal tip to reduce swelling in that area within the first several months after the surgery.
