Can a intramuscular lipoma on lower back/iliac crest cause severe pain?How long until I am back to sports after removal surgery?

I have a deep lipoma on my iliac crest and lower back.My orthopedic surgeon that performed two arthroscopic for a torn labrum, said he would suggest an injection.I am in a severe amount of pain and he does not seem to want to remove it.The pain is a shocking sensation on my lower right back, weakness to front of groin and down my right leg.I consulted with a dermatologist,he told me it was intramuscular and a general surgeon is a good option.(The lipoma did not show on a mri but you can feel it)
