I now have inverted nipple grade 1 because I bottomed out. I had implants for approx 20 years , due to weight change and loss of structure I bottomed out. I now permanently can not regain an erect nipple . Why would my nipple get damaged because of implant bottom out?

With shoulder and neck pain, you need to go down in size because the implants are too heavy for your frame. With bottoming out, you also need to go down in size because your tissue is not supporting this weight. An internal bra of mesh could be very helpful to give your tissue more support. ...
You are a nice size now even though the left is slightly smaller. I would suggest not adding a cup for your tissue may not support the additional weight after being stretched and you may bottom out like the girl in your wish photo.
You need to go smaller and lift again. With such heavy implants that are too large for your frame, they will not stay where you want them. You may also need mesh for better support.
