Question

I now have inverted nipple grade 1 because I bottomed out. I had implants for approx 20 years , due to weight change and loss of structure I bottomed out. I now permanently can not regain an erect nipple . Why would my nipple get damaged because of implant bottom out?

There is a risk of damage to nipple sensation and erection with breast surgery. This is not fixable since the small nerves have been damaged.

Nipples tend to be quite unique, and some people will have nipples that behave differently on both sides.
Change nipple sensation is fairly common after breast surgery.
To make an assessment regarding the outcome of your best augmentation we need to see standardized pictures.
Take a look at how plastic surgeons take before and after pictures to get an idea.
 it would be unlikely for someone to bottom out implants 20 years after the procedure.
Developing breast ptosis would be far more likely.
Consider following up with your plastic surgeon or schedule a second opinion consultation with other providers in your community.
