54 year old with 30 plus year old smooth silicone implants with a possible rupture under muscle. I have heard and seen the best results in photos from the J incisions versus other types based on my attached photos what are chances of being able to do the J incisions versus other methods
Answer: Explant with Bellesoma Method The technique I recommend is explantation with lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided and nipple sensation (in 95%) is maintained. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: En bloc removal and mastopexy A removal of the implants with the capsule and a mastopexy would be ideal in your situation. A vertical mastopexy would limit scars and give you a nice breast shape.

Answer: What are chances of being able to do J-incisions lift with under muscle removal? Hello @Thriving215562, thank you for your question. It is possible a L incision to remove under the muscle prostheses. The incision really depends on the technique used by the specialist who is going to performed the explant. I would suggest a breast reconstruction with reposition of the tissues and in this way achieve an aesthetic result that is adequate with minimal scarring like. Due to the experience of so many explantation surgeries practiced in these 21 years I consider that the L-shaped incision is the minimum and smallest scar possible in this surgery. The best course of action is to consult one or several board certified plastic surgeon for more information and options tailored specifically to your needs. Best wishes! Alan Gonzalez MD, FACS.

Answer: Implant removal Dear Thriving215562, it is hard to tell for sure without an examination. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations. Answer: Breast Lift Seeing a board certified/eligible plastic surgery would be the best option to do an exam and speak to you in depth about your surgical goals. Things to consider is if you want to completely remove the implants and undergo a lift at the same time or if you plan to get new implants. If you would like new implants, would you be keeping the same size or going bigger or smaller. If you remove your breast implants completely there would be slightly more excess skin that could need more skin tightening (i.e. an anchor scar). If you keep your implant the same then less of a lift would be needed, although a small anchor scar still might give a better result. A consultation would be needed to better evaluate the amount of skin laxity and talk to you more about your goals.
