Hard small lump deep in left buttock after BBL 4 years ago - should I be worried?

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

And ultrasound is going to give a very non-specific result. It can usually differentiate solids versus liquids and tissues that have different density.

With a history of a BBL this almost certainly represents fat necrosis with the scar tissue that it creates.

It can also be an oil cyst also from fat necrosis.

Both of those come from surgical technique when too much fat is injected into a single small space.

All fat transfer procedures create a fat necrosis but if the fat particles are diffusely spread out they are small enough to not become symptomatic.

I suggest you follow up with your plastic surgeon who did your BBL procedure. That person would be more appropriate to review your concerns then your primary care Doctor Who didn’t do your BBL.
