I’d like to eliminate the small excess bump of extra fat under my chin however I’m not sure if liposuction or a different outpatient approach would be appropriate. I’ve had consultation at “med spas” which have Kybella and different skin tightening treatments available. However, the waitlist for a plastic surgery consult at my medical center is quite long so I have admittedly not yet met with a plastic surgeon on this issue, but plan to do so. I’m 31 years old so I believe my skin laxity would still be enough for lipo. However, I don’t have a large amount of fat here and I’d like a conservative result. I’ve seen some commentary on celebrities who have emphasized platysmal bands due to (suspected) chin lipo as well as concerns about “trending” procedures which remove facial fat causing an unwanted aged appearance. Are the aesthetic risks worth the reward in this situation?

Answer: Chin Contouring Options: Liposuction vs. Non-Surgical Treatments for Submental Fat When considering options for addressing a small excess of fat under the chin, both liposuction and non-surgical treatments like Kybella or skin tightening procedures can be viable options, each with its own set of benefits and considerations. At 31, with presumably good skin elasticity, you have a few options to achieve a conservative result. Chin Liposuction: This surgical option is effective for removing localized fat deposits and can provide precise contouring. Given your concern about a conservative result, liposuction can be tailored to remove just the right amount of fat. However, it's important to consider potential risks such as the prominence of platysmal bands or changes in facial contours, which can occur if too much fat is removed. Kybella: This non-surgical injectable treatment gradually dissolves fat cells under the chin. It's a good option for those looking for a less invasive approach, although it typically requires multiple sessions to achieve desired results. The gradual nature of the treatment can lead to a more subtle and conservative outcome. Skin Tightening Treatments: Options such as radiofrequency or ultrasound-based therapies can improve skin laxity and may be used alone or in conjunction with fat reduction techniques. These treatments can enhance skin firmness and contour without surgery. Ultimately, the choice between these treatments depends on your specific anatomy, aesthetic goals, and how conservative you want the outcome to be. Consulting with a board-certified plastic surgeon or dermatologist will provide you with a comprehensive evaluation and help determine the best approach for your needs. They can discuss the potential risks and benefits of each option, ensuring the aesthetic risks align with your desired results.

Answer: Choosing the correct treatment You have a well developed mandible, and your subcutaneous fat layer is quite thin. For that reason, you have good jaw line definition, and I would recommend you do nothing.Liposuction is a risky procedure. If you don’t find the right provider, you can be left permanently disfigured. Thinking you know how to find the right plastic surgeon is overly optimistic.Finding the right providers is extremely difficult and patience with good intentions who thought they had “done their homework” still end up with horrific results and go hiding in shame.I don’t wanna come across as alarmist or overly pessimistic, but surgery should be avoided, unless there’s a clear indication.Mastering Liposuction is more difficult than most people realize. The number of plastic surgeons who have mastery and Liposuction is in reality quite small. It may seem like a simple procedure, but it is not.Because you don’t have much fat to remove the impact, it’s going to be subtle.Still, the risk is higher, especially of over treating.I don’t like Kybella. I believe Liposuction is a better option if you want to have that removed.Any form of non-surgical skin tightening is going to leave you with invisible results. The idea of tightening skin without surgery is the holy Grail that still has not been found. If you were a man, I’d tell you to stop looking in the mirror, but you’re not so I won’t.You look great and don’t need surgery.Best,Mats Hagstrom MD
