I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

You got a very good result with a difficult operation. Slightly different nipple heights. I would not mess with it.

You had pre-existing large breasts. You would have been better served with implants that are smaller, and focused more of tissue lifting and tightening. A long IMF to nipple distance is the result of the 415 cc implant. In order to correct your current saggy appearance, seek the care of a plastic surgeon who specializes in mastopexy, and discuss your options. You should have full confidence of an improved result.

The anchor lift technique is incapable of lifting the center mass of your breast. The lift technique you needed was The Bellesoma Method. Unfortunately, at this time, you have lost too much skin that you cannot have The Bellesoma Method done. At this time, I recommend removing the implants, place fat transfers to the upper pole portion of the breasts and revise the inframammary fold of the breasts, advancing the tissue underneath.

First of all, "between natural and fake" is not a thing. I don't know where you came up with that, but there is no way to put a definition on it. Secondly, a breast augmentation/mastopexy, as I have mentioned many times on this site, is one of the most difficult plastic surgery procedures because it brings the variables and risks of 2 different surgeries to the table at the same time. Finally, what is photo #2? If that is your early postop photo the result looks phenomenal. Which means that the current suboptimal result is a result of your breast healing and continued aging, i.e. gravity. A breast lift revision may be in order now. Good luck!

Yes, I can see a difference between both breasts, and yes, better results could have been obtained. After 9 months is enough time to consider visiting a qualified and certified surgeon, discuss exactly the results you would like to obtain.

The ideal, however, is that enough time has passed for the operated area to have completely recovered from the first intervention, usually 1 or 2 months minimum. In general, the same scar that was made in the first operation is used, so it is advisable that the skin and muscles have recovered before incising the area again.
