I told my surgeon size wasn't necessarily what mattered with my procedure but I was more interested in a specific look. I wanted round breast that sit higher up on the chest. I wanted it to appear I was wearing a push up bra even when I didn't have one on. He told me lift was not necessary to get my desired look. I trusted doc and now I'm unhappy and they tell me the only way to fix the sagging is with lift? Is there any exercise I can do to have that cleavage look toward the top?

I would recommend switching the implant to below the muscle and performing a breast lift to address the residual excess skin/stretched out breast soft tissue envelope. You would likely benefit from some type of scaffold or mesh support to provide extra stability for the implant in its new pocket.
