the recommendation for max volume removed is 5L per surgery. my whole practice is doing lipoabdominoplasty, or at least liposuction with internal heating (which is not the gold standard for skin tightening with body contouring).the smaller you are before liposuction the more contouring you can get from the procedure. most of my liposuction patients get pre and post surgical optimization with hormonal therapy (see link to help boost their weight loss before surgery and help them heal after).You need a formal evaluation by an expert in body shaping and contouring to determine what is best. Traditional liposuction awake and smart liposuction are old methods and do very poorly for tightening skin or significant skin tightening or contouring. Small problem areas can be improved slightly by these methods. The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heat the skin with radio-frequency devices such as J PLASMA/RENUVION, InMode BodyTite, ThermiRF to heat the under surface of the skin and tighten it at the same time as fat removal. If skin is very loose before surgery then it will need to be removed such as lower abdominal skin (tummy tuck/abdominoplasty), breast skin (breast lift), arm skin (brachioplasty). Since early 2000’s the Florida Board of Medicine places a listing of rules/requirements/recommended practices for office surgery. As for liposuction- the max amount of supernatant fat is 4 liters. If doing any combination surgery as in TT or breast surgery, etc only 1 liter supernatant fat is allowed. That is rule 64B8... As for doing in hospital operating setting, each hospital sets amounted allowed to liposuction. Usually based upon blood losses. I have seen from 6 liters to 10 liters of liposuction in a hospital Operating room setting, this is Large Volume Liposuction. So to be clear maximum LIPO in office is 4 liters of “fat”! If combining with other surgeries only 1 liter of “fat”...

I am afraid your inquiry appears to show little understanding of why there are laws or rules about the amount of fat removed during liposuction. Liposuction is, in fact, one of the most dangerous cosmetic procedures done. People can die from liposuction either at the time or over the next several days after the procedure. Unfortunately, many improperly trained physicians who do not understand the body's physiology during and after liposuction begin doing it because it looks easy and, with only a few small incisions, looks safe. The problem is that those little incisions have nothing to do with the body's reaction. When you remove the fat, no matter what method you use, you are significantly injuring the area. This is akin to creating a 2nd or 3rd degree burn of the skin over where the liposuction is done, only more severe. The body reacts by pouring intravascular fluid into the injured area. If it moves enough, you go into shock, just like losing a lot of blood (which is what you are doing, losing the fluid portion, just not the cells.) It also calls on its healing systems and produces multiple hormones and steroids, all of which can cause reactions throughout the body. The thicker the fat removed, the more reaction. The broader the areas, the more reaction. Liposuction of a small area, as with a small burn, can be treated at home by drinking GatorAid or something similar to replace the fluid lost. With larger burns or larger areas of liposuction, you require hospitalization and IV fluids. Additionally, the more you liposuction, the more anesthesia you need, and the anesthesia can be toxic. The laws were created in many states because people were dying either during or after liposuction in an attempt to prevent any more deaths. Since they were made by non physicians, they are only guidelines and are imperfect. This is why it is critically important that you have your liposuction only by a board certified plastic SURGEON who understands the body's reactions and physiology and the medications used for anesthesia and sedation. If that person does not want to do as much as you desire, listen to him/her. Your life depends on it.
