I have been having more pain than usual on my right breast. I have had a contracture for about 5 yrs now on that side. Today I noticed what appears to be dimpling right where Im having pain. When I move my arm, it almost feels like the implant is sliding against my muscle. Very uncomfortable. What could this be? And why is it concave like this?

At 5/6" you might want to consider 300-350 cc's, taking you down a cup, or the smaller may need to be even around 250-275 cc's. Each cup is about 150 cc's. The size depends upon how much actual breast tissue you have.
You need more tissue, so using dermal grafts like Alloderm could be helpful to give more tissue, and/or fat transfers in the lower breast. Later, there are some lasers to help decrease the discoloration from the veins.
The 375 cc implants are too large for your frame and tissue. You would probably do well with 200-250 cc implants, and be much more comfortable with less weight on your neck and shoulders.
