I'm a male, and a natural 42B, and I want to go up to a D or DD. I don't want any transgender surgery; I just want female breasts. (I'm aware that this would give me the appearance of androgyny.) Would you recommend such a procedure, and if so, would it be easy/difficult to find a surgeon who could perform the operation, seeing that I'm male?

Answer: Male breast implants are possible. I have done this surgery for several men who are not identified transgender patients, but also for many M-F transgender patients over the past 2 1/2 decades. I'm comfortable with the male anatomy and the differences this causes in the surgery to place breast implants.For example, to go to the size you are requesting is indeed possible, but to give you the best cosmetic result, you may require a two-stage procedure with initial placement of tissue expanders to stretch the lower poles of your chest/breast tissue. After outpatient expansion is carried out, the expanders are removed and the proper large implants are placed.Otherwise, simply cramming in large(r) implants usually doesn't work, and when tried, yields too-small, too-tight, wide-cleavage, convex upper pole and concave lower pole breasts that look unnatural and are often firm or capsular-contracted. Single-stage augmentation is possible in some patients with adequate skin and muscle laxity, but the male anatomy and physiology does not always allow implants (of the necessary size to give you the volume requested) at a single stage. You will find some surgeons who are uncomfortable with what you are requesting, maybe even most. But as these answers show, there are indeed some of us who will do whatever is in the best interests of our patients who have reasonable requests, expectations, and understand the potential risks and complications. Sometimes, a large size is difficult to achieve because the skin is tighter and may not allow for a very large implant. Even if that's the case, the implants can be changed out for a larger size in the future once the skin and tissue has stretched. Please consult with a board certified plastic surgeon.

While many male patients undergo a range of transgender procedures to achieve complete gender reassignment, such a journey is not for everybody. You have freedom to choose how far (or not) you wish to proceed along that path. If your stated goal is to acquire female breasts through implants, but not to undergo any other treatment (hormonal or surgical), there is nothing wrong with that. It should not be too difficult to find a competent and sympathetic surgeon to accommodate your wishes. However, I do recommend that you receive appropriate psychological counselling from transgender specialists and that you are fully informed as to the pros and cons of such a step, before you commit to the surgery.

Answer: Implants in a male can certainly be done and you can simply call your local plastic surgeons to see who would accommodate your requests. Its certainly easier to do this in true transgendered but if your motivations are legitimate and important to you, I'm sure you could find a surgeon relatively easily.
