It is difficult to adequately assess your concerns with these pictures alone.  Some procedures that can improve buttock shape and remedy loose skin include a lower body lift, a buttock lift, an autoaugmentation, liposuction with fat grafting, and / or a combination of these.

In your particular situation buttock lift is necessary to tighten the skin and provide some volume in the upper buttocks. This can be done using incision that is similar to abdominoplasty incision anteriorly. Instead of discarding lower back/upper buttock skin and fat, this tissue can be buried under the buttock flap to create significant bulk in the upper buttocks. Additional fat grafting of the middle and lower portion of buttocks would create desired shape and volume. In person consultation with an experienced board certified plastic surgeon is recommended since this procedure is not frequently performed. Good luck.
