Question

Good day, I'm a 36 year old male who lost 150lbs apprx 5+ years ago. I've already had Lipo+Renuvion on my neck about a month ago, and I'm assuming this wont get much better. Is it possible to do a wedge cut // Submentoplasty to remove enough skin/tighten under my neck to fix this issue? If I pinch two sides under my chin it goes away, so is there a way a surgeon could strategically cut out some skin under my chin to cause the same effect?

Providing that you can tolerate a long scar in your anterior neck, direct web/fold excision with resulting Z shaped scar is a good and simple option for current skin redundancy. Good luck.

Thank you for your questions and photos. 
One thing I will say about the renuvion procedure is that results can improve over the first 3-4 months so there is a chance this will improve with time, though how much is hard to predict. Regarding your question about excising skin, there is a procedure known informally as the "graecian urn" where it does just that by cutting out excess skin, however it can leave an unsightly scar so I would strongly suggest looking into what that looks like before signing up for that procedure. The final option would be something like a neck lift procedure where I would likely combine a small incision around the ear lobe and behind the ear to redistribute that excess skin.
