Should I repeat my blepharoplasty or only do an eye browpexy? (Photo)

Question

I had upper bleph 2 years ago, however I feel my eyelids are still hooded. I’ve been told my brows sit in a low position which could contribute to hoodedness. I went back to my surgeon to inquire about brow lift. He recommended a browpexy and gave the option to remove a little more eyelid skin. I’m really torn on just doing the browpexy or going ahead with another bleph. I would love more eyelid space for makeup but worried it would make my eyes look worse. I measure my crease to be ~5mm

Your eyebrows are asymmetrical, one is lower than the other. You may benefit from a brow lift to correct the asymmetry. This may also improve the upper eyelid skin appearance. I would recommend you do the brow lift first and then if you are not satisfied then repeat the upper blepharoplasty.

In my hands a browpexy is more of a stabilizing procedure rather than a lifting or recontouring procedure. If the goal is to lift the brow I think a browlift is a better option. You could also try neurotoxin to see if you can achieve a chemical lift.

To answer this question, you need to be physically examined by a surgeon. Anyone who claims they can answer this question based on these photos are pulling your leg. Generally browpexy is an ineffective procedure. It is done as an add on procedure at the time of blepharoplasty. A stitch is placed between the brow and the bone covering. Sadly, this single stitch just does not hold up. I don't advise this. Your prior blepharoplasty likely did not provide a strong adhesion between your anterior levator aponeurosis and the new eyelid crease. Without a proper anchor blepharoplasty, the eyelid will lack the definition needed. It may be that you need a properly performed anchor blepharoplasty. On the other hand, there are individuals who really do benefit from a forehead lift. In that case it should be done endoscopically. This can be determined by a detailed in person assessment . Your eyes can be your best feature.

Based on your photos, you may have a drop of excess skin in your upper eyelid creases. If this is the case, please consider meeting with an oculoplastic specialist to remove a drop more skin, if this is bothering you. I hope this is helpful! Sincerely, Dr Joseph

I’d be careful removing more skin from your upper eyelids.
Once it’s gone, it can’t be replaced.
To make a quality assessment typically need to see a complete set of proper before and after pictures.
If you don’t have a full set of before, and after pictures and ask your surgeon to forward the pictures they took.
 Maybe you don’t have to do anything?
