Ever since I was around a mid teenager I've always pulled my hair back to create a tight cat eye look but not cat eye to the point of it looking crazy, in the photo of the girl I've provided I've shown a desired goal. I want to know what surgeries I go about to get that end result.

In most cases, achieving the cat eye/almond eye look involves performing two procedures in at the same time, a lateral canthopexy or canthoplasty and a lateral brow lift. A canthopexy is a surgery were the outer portion of the lower eyelid is lifted using sutures, this will give you a cat eye look and will change the eye shape. A canthoplasty is another way of doing it that is a little more involved. A brow lift is coupled with the above procedure to ensure that the brows match the new eye shape. That being said I am not sure if it is the angle the picture was taken at or if you truly have upper eyelid droop , but it looks like you might have some element of upper eyelid droop with already elevated eyebrows which might need to be addresses as well , but for a tailored plan and better assessment I would recommend that you visit your plastic surgeon for an in person consult. When the ptosis is corrected, the eyebrows will relax into a more natural position and you will have your look. Please be aware that there are two types of ptosis surgeries: anterior levator resection ptosis surgery and posterior approach ptosis surgery, which is also called Mueller's muscle conjunctival resection ptosis surgery. In my experience, the posterior approach ptosis surgery is not as effective as the anterior levator resection ptosis surgery and does not result in as much change in the eyebrow position. Please understand that many eyelid plastic surgeon do ptosis surgery and they all do blepharoplasty but very few specialize aesthetic reconstructive eyelid surgery, which is what you need. Answer: Changing your eyes The wish picture is not really achievable. The major difference is centhal tilt which is based on bone structure. You could consider ptosis surgery. Best, Mats Hagström MD
