I’m 36 years old and my lower lids/upper cheeks wrinkle a ton when I smile. Botox in the crows feet area isn’t addressing it. Do I need a lower bleph even though it’s not bulgy?

Hello, I am Dr. Cemil Işık, you have identified your problem well. The treatment for this is only possible with surgery. The tight eye connections we call "canthus" need to be repositioned in right place, a little higher. This surgery is called canthoplasty.
Complication rates need to be put in context of not the most serious complication, but rather the frequency.
The most serious complication of any surgical procedure is death.
That doesn’t happen very often for elective cosmetic surgery.

Your eyebrows are asymmetrical, one is lower than the other. You may benefit from a brow lift to correct the asymmetry. This may also improve the upper eyelid skin appearance. I would recommend you do the brow lift first and then if you are not satisfied then repeat the upper blepharoplasty...
