You may have right orbital fracture with resultant right eyeball sunken and down (enophthalmos). You need to get orbital CT scan.  You may be candidate for orbital reconstruction with implant. See link below.

Hello.  Thank you for your question.  The picture provides a limited view of the situation.  However, from what you are describing, it sounds like you likely sustained a zygomatico-maxillary complex fracture with associated orbital floor fracture.  This can result in depression of the zygoma (cheek bone) and potential herniation of the orbital contents into the underlying maxillary sinus, which can allow the eye to sink into the orbit with resulting orbital dystopia (one eye lower than the other) and entrapment of the extraocular muscles that move the eye.  This can certainly lead to double vision.  This is a complex situation that needs to corrected.  I would strongly suggest that you seek consultation with a board certified Facial Plastic Surgeon with experience in managing facial trauma.  I wish you the best.  I hope this information is helpful.

Your photo is inadequate for diagnosis but your description is excellent. You may need a surgical procedure to reconstruct the orbital floor. Please make sure to see a surgeon that has a great deal of experience in the procedure, many surgeons do not.
