Double chin: Would I need 2 vials of Kybella based on these photos? (photos)

Dear sophiaxd_,Kybella typically takes 3 to 5 treatments to work. This is why I've stopped carrying Kybella in my office.  I recommend neck liposuction more often then any injectable procedure for the neck now. It provides superior results gives me more control over the area than any injectable.  It also costs about the same as what you paid for your first kybella session. You can have up to four vials of Kybella during each treatment. Most people need about three treatments, total, to get the fat-elimination and skin-contraction they’re looking for. However, two vials will start to eliminate your excess fat in that first session. Consulting a board-certified dermatologist will give you a better sense of what you actually need, rather than be limited by a daily or weekly special.

From the photos it appears two kybella should make a difference. It is all about what bothers you exactly. So be precise.  In my hands I get better results with liposuction and jawline sculpting ( I do kybella also) but I am a surgeon so maybe I am jaded in that way. Below is my typical approach as well as the attached video for some instruction. This area can have persistent fat in even very thin patients. So it is all about having fat in the right places. Sometimes we even move this fat to another area to help define the jawline or cheekbones. If you are not interested in moving the fat then we can use fillers such as restylane of juvederm to give some definition along with sculpting the jawline and neck. We would need to do a good physical exam on you to determine exactly but we typically remove that layer of fat beneath the skin and allow the skin to fibrose to the muscle of the neck.  It is not uncommon to combine this procedure with other fat sculpting techniques such as buccal fat removal. The muscles of the neck and tongue in conjunction with the only free floating bone in the body (the hyoid bone) are the limitations of what you are trying to obtain. After palpating that bone and those muscles you could get a better idea of what can be obtained.

You'll need an in-person evaluation to determine how much Kybella will be needed. Both Kybella or neck liposuction are good options and will improve the double chin. It depends on your lifestyle and personality which would be better for you. 
Kybella is best for patients that:
1)  Do not want surgery 
2)  Are not surgical candidates 
3)  Not willing to wear a compression dressing around the face for 1 week 
4)  Patient-  The results of Kybella will take months
Kybella take 1-3 treatments every 6 weeks.  Patient will develop swelling and possible bruising after each injection that may last a few days.  The benefit of Kybella is each treatment is quick and completed in the office. However, some patients need more than one treatment to get desired results. 
Neck Liposuction is best for patients that:
1)  Want a single treatment 
2)  One recovery time-  Downtime for neck liposuction require wearing a wrap around face for 1 week. 3)  Less painful 
4)  Quicker results 
5)  Cost is a concern
Neck liposuction is a quick surgical procedure that can be complete while the patient is awake.  It cost around the same price a Kybella and can be less in patients with heavier necks.  The single recovery of neck liposuction may be a better option for some patients.

Newer algorithms for Kybella are 2 vials spaced 2 months apart and in your case I suspect you'll need between 6 and 8 vials total.

Kybella is an excellent option of treatment of the "double chin".  In my practice I have found that the average treatment volume is 2 vials.  Most patients will also need 2-3 treatment depending on their response and their goal contour.  Typically these treatments are about 6-8 weeks apart.
If you have resultant excess skin there are adjunct procedures to help with that issue - however due to your age and the ability of the skin to retract there is a good chance you would not need anything additional.

However it is highly improbable that a single service will get the job done.  It is much more likely that you need to have 5 or 6 of these services at 4 to 6 week intervals to accomplish your goals.  So multiply that price by 5 or 6 to know the actual cost of accomplishing the goal of improving the chin area.  Even then, many patients are still disappointed with service.  The treatment is very uncomfortable and associated with persist swelling that lasts weeks.  Some experience numbness and muscle weakness.  Read the information about this service to decide if it is right for you.  I personally think the treatment is a bummer and do not offer it to my patients.  Let the buyer beware.

This needs an in-person evaluation, but yes, most people need more than one treatment with Kybella. Also, Kybella is good for dissolving fat under the chin, but a combination approach with other treatments is needed to get the most improvement with non-invasive options. I recommend Kybella with thermage, ultherapy, exilis. VASER liposuction in combination with ThermiRF is the best alternative for contouring the chin, if you're a candidate. I never promise a certain number of vials of Kybella will work for a patient. After performing Kybella over several years, what I have realized in the response is variable between patients. In most patients 2 vials will produce some improvement but not the full response they are looking for. 
In fact studies have show for maximal improvement and satisfaction, 3-5 treatments are necessary. Good luck!
