What can I do about TMJ that caused a wide and sagging jawline? (Photos)

I am 23 years old and had TMJ for many years of my life that caused the muscles in my jaw to enlarge. I am actually on the underweight side according to my BMI but my face looks so unchiseled and round. I was looking into chin lipo, buccal fat removal, chin filler possibly. I didn't see much of a difference when I got 40 units of Botox a year ago, it was more of a physical release of my jaw but maybe I should get more?

After a thorough examination, a board certified plastic surgeon can determine if you are a candidate for orthognathic surgery. If not, chin liposuction may be appropriate and may make a nice improvement. The problem is that there are other things in the neck that need to be evaluated including the underlying muscle and fat underneath that muscle. Liposuction can only remove the fat above the muscle; FaceTite can stimulate collagen production and diminish skin laxity if necessary. Consult via email or Skype with a board certified PS with alot of experience in chin surgery.

it is hard to tell for sure without an examination. Based on your photos, it looks like you would benefit from a chin liposuction. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

I don’t think your TMJ problems are causing your chin to look the way it does.

It’s probably more related to the shape of your mandible.

To make an assessment we need standard full face pictures from the front inside without facial expression.

Full face is needed because we need to compare the overall balance between the maxilla and mandible.

Most likely Liposuction is your best option for what you described.

I’m reluctant to recommend buccal fat removal because of the implications it can I have as patients get older.

I suggest scheduling a few in person consultations for local local board-certified plastic surgeons.
