One year after a revision surgery, my nose formed a bone callus on my dorsal hump and my nasal tip is still droopy and bulbous with a scar tissue bump on it. Is there anything I can do non-surgically lift/shrink the tip, remove the scar tissue and lessen the appearance of the bone callus? I have had two revision surgeries already, (the first resulted in scar tissue and the second surgery to remove the scar tissue, resulted in my septum collapsing. The third surgery was to fix the collapse).

Answer: Rhinoplasty revision There are two things you might consider and both can be perform easily under local anesthesia. The swelling in the go can be reduced with a series of steroid injections in the tip. This will thin out the skin and scar tissue and give better definition. If there is a bony irregularity on the bridge it can be filed smooth with a very tiny file specifically designed for your situation. The file is 2 mm wide and used Incase’s like yours. If you want the tip raised however this would require surgery.

Unfortunately, there is no non-surgical way to achieve your goals. You could try to hide the residual hump with filler and you could try to create a "tip flip" effect with filler, but this is a temporary fix. Looking at your nose, it is clear that your surgeon left you under-rotated and long. Depending on the amount of scar in your nasal tissues, this could be fairly easy to fix with judicious tip skin release and caudal septal length reduction. The hump can be filed down with a micro rasp or a piezo device. Once this is done, the tissues can be treated with dilute steroid to reduce the incidence of scarring post-op. I can imagine that you are pretty fed up with having to have surgery after surgery. I would recommend that you consult with a surgeon who specializes in revision rhinoplasty to achieve your goals.
