Undergoing a rhinoplasty should not aggravate your spider veins.  I would recommend discussing all of your concerns with your surgeon that you have surgery scheduled with.  Good communication between patient and surgeon is very important both before and after surgery.

 The spider veins can be cauterized in the office setting prior to the surgery. Best not to zap them during the surgery itself. The surgery could potentially aggravate them and make them worse, so it best probably have them cauterized prior to the surgery. A septoplasty is performed to improve breathing, while  a rhinoplasty is performed to shave down the dorsal hump. A rhinoplasty will not improve breathing. Rhinoplasty surgery will not affect your spider veins.  You could consider having those removed by laser surgery.
