Hello and thank you for your question.  I would absolutely recommend removal of the piercing before your surgery.  It can conduct an electric current to cause a skin burn, and can also increase the risk of infection.  The next step for you is to find the right aesthetic plastic surgeon.  In your rhinoplasty consultation, be sure your surgeon addresses all your aesthetic goals and concerns specifically, and discusses the surgery details including the position and size of the incisions used (with actual photographs and not just digital animations), the risks and benefits of the procedure, recovery time, and post-operative course.  Also, be sure to see plenty of before-and-after pictures in women who have a similar nose appearance to yours, and preferably at different time frames (3 weeks out, 3 months out, etc) to more accurately predict your own surgical outcome.

After seeking out a board-certified plastic surgeon who specializes in aesthetic plastic surgery, I think the most important component of the consultation is to establish a good connection with your plastic surgeon.  It’s the connection that allows you to clearly define your desired nose outcome goals, the first step of this whole process. If you don’t feel you have a good connection with your plastic surgeon after your consultation, he/she may not be the best surgeon for you.

I cant tell where the piercing are. If they are in your nose they come out for sure. Otherwise it will be up to the discretion of your surgeon. I have never seen or heard of a burn from jewelry left in during surgery, but most surgeons will want them out.

If any portion of the piercing is metal, it should be removed. You need to also consider the swelling that will occur and the possibility for infection.

In my opinion, your piercing would need to be removed during your rhinoplasty surgery in order to prevent harm to you.  I would recommend discussing this with your particular surgeon.

Thank you for your photo. Your tragal piercings should not be affected by the rhinoplasty as most rhinoplasty procedures do not require an electrocautery. In the case that it does, a bipolar electrocautery can be used instead which limits the spread of energy to the area being treated so you are not at risk of metal burns. Hope this helps!Johnson C.  Yes, it is possible to keep the ear piercing in place for rhinoplasty surgery.  In our practice, we do not use cautery for a rhinoplasty procedure, so metal piercings are irrelevant for that   Specific procedure.

If for any reason, electrocautery is required during the operative procedure, accidental contact with (or often just close to) a cutaneous metallic piercing may cause a 3rd degree burn ( or even a fire), resulting in potential significant scarring (the RF energy can penetrate tape). In our practice, we usually advise removal of the pin on the morning of the procedure with replacement the very next day. 
Some surgeons will allow temporary replacement of the metallic piercing with a non-metallic device or a suture to maintain patency during and shortly after the procedure. The jewelry is quite attractive. That 90 degree bend, however, would be difficult to overcome.  If true, it certainly wasn't manufactured in a user friendly fashion. How was it initially inserted? If with a needle or gun, it will be an oblique penetration rather than a sharp bend, and replacement may be easier than you think (if in place for more than a few months to allow for tract epithelialization).
Discuss this issue with your plastic surgeon.
