My nostrils are too uneven and wide. I don't like how much nostril shows from the front view. Would Alar Sill or Weir Excisions help my nose? Rhinoplasty/revisions to try and fix my nose & now considering Silikon 1000 & Alar Base or Sill Surgery. #1 Concern is nostril show too much from front. Nose didn’t have this prior surgeries. #2-3 Concerns are nostril irregularities and dents/divots in alar and lateral cartilage. Not interested in temporary fillers

It does appear from your photos that revision surgery with a highly-qualified surgeon could produce better results for you. Keep in mind that revision surgery is even more complicated than the original procedure, since there may be grafting material and scarring to deal with on top of the necessary corrections. By Closed and Atraumatic technique a scar on your columella will be eliminated, your healing will be maximal with minimal edema&swelling. Regards,

Answer: Fixing excessive nostril show You may get differing opinions on this, but the reason for nostril show in your case is that your lower lateral cartilages are either weak or deficient causing a notched appearance. This is most commonly seen after rhinoplasty when there has been an excessive cephalic trim of the lower lateral cartilages. In your case, this is compounded by the fact that your alar base is lower than the columellar base. This will give someone the appearance of flared nostrils. In this setting, a conservative Weir excision can help bring the balance of the nose into better harmony. For the excessive nostril show, I typically will reorient and strengthen the lower lateral cartilages with alar batten grafts to improve the appearance and function of the nose. This will also take care of the unusual notching (parentheses) you see in your tip. Just a word of advice, please be careful when trying to Google answers regarding something as complex and individualized as rhinoplasty. There is no single maneuver that will apply to a specific issue in rhinoplasty and especially revision rhinoplasty.
