I am desperately wanting a lower facelift and chin implant. Photo have poor lighting (sorry), but I think get the point across. I have 8 syringes of juvaderm (mostly Voluma and Volux) across my lower chin and cheeks (one Volbella in my lips), so my normal profile is nowhere near this. $8-$10k of fillers nd Botox per year and I think it's time to just make some surgical changes. What would you recommend? I'm wanting a big change that will last me hopefully until 50 (I'm 38 now.)
Answer: Chin implant There are a couple of treatment options in your case but I would not suggest a lower face and necklift. With all the filler that has been injected your chin projection looks adequate. If we assume that is the result of 8 syringes of filler then you probably can maintain that projection with a chin implant. In the side photos, It appears that your jaw angles are not projecting outward enough so I think jaw angle implants may be beneficial as well. This will create a stretch on the tissues and diminish the appearance of jowling and improve your chin-neck angle. A short incision facelift would also be an option to reduce the lower facial jowls. The cervical mental angle will improve with the chin implant.

Answer: A lower face/neck lift does give a more pronounced chin/jawline. A lower face/neck lift is easy to undergo under local and gives that great sharp jawline which usually can make the chin look more pronounced as well.

Answer: Chin Implant and Deep Neck Lift Based on the photos shown, I think that a chin implant will provide benefit, assuming that you are a healthy nonsmoker. A deep neck lift will address the obliquity in the neck but will not address the lower face quite as well. A deep plane facelift might be a little aggressive at this point, but that's something that would require a consultation with more information to determine.

In my opinion, a combination of deep plane facelift and deep plane neck lift is your best answer for significant and lasting changes. You may need to reduce some of the filler volume in the cheeks as the redistribution of volume from the lower face towards the midface is quite significant if you do a deep plane facelift intervention.
