Lipo on lower/inner butt? Can lipo fix this or only a butt lift- scar in crease? (Photo)

Obviously, an in depth exam is of paramount importance, however, based on your picture,you are actually an excellent candidate for an infragluteal thigh lift which I term the "Del Mar Butt Tuck". This is a difficult problem to fix and make it look like your "wish" picture. In the wish picture, the buttock crease is short (extends to the mid posterior thigh) and it is at a 45 degree angle. In your case, the crease is horizontal, almost tilting up at the ends (unfavorable), have fullness of the medial buttock cheeks and medial thigh. The aim is to create a diamond shape space between the buttock cheeks and the medial thighs bordering the space. I would approach this with liposuction of the medial buttock cheeks and upper medial thigh to create the diamond shape space. You will also need fat grafts to the lateral lower buttocks to alter the length and angle of the crease. There is a possibility that skin excision with some fat needs to be done in the medial buttock cheeks, with the scars hidden in the medial crease at a later date.
