I had transconjunctival blepharoplasty 2.5 months ago & my eyes look round! I am very concerned I may not go back to almond shape & stay with round eyes which make me look different. The bags are completely gone which is great.. Is there still a possibility of my eye shape going back to normal. Is there anything I should do to help like cold compress or warm, eye excersice (if so how do I do). My dr just keeps telling me to give it time 6-12 months etc. I am getting anxious because I see little to no change in shape. Your input would be very appreciated.
Answer: Postblepharoplasty lower eyelid retraction It is very unusual for transconjunctival lower blepharoplasty to cause eyelid retraction. Need proper evaluation with expert oculoplastic spcialist for lower eyelid retraction and possible treatment with lower eyelid retraction surgery.

Answer: You are not that early after this surgery. Looking at your before photograph, in my opinion and in my practice, I would have recommended under eye hyaluronic acid filler to soften your under eye circle. You really did not have herniated orbital fat. In going after the minimal fat that was present, there is now a collapse of the lower eyelid position and a loss of the youthful fullness of the lower eyelid. As time passes, this effect will not get better. It is more likely to get worse. I suspect that you could get considerable help with again a properly done, fussy under eye filler treatment, It may not push up the lid margin but it can. It will address the hollow. It is possible to do a much more elaborate reconstruction of the almond shape of the lower eyelid that that is a very expensive process that generally requires two surgeries. I would start with the simplest step that is likely to help and proceed from there. A detailed personal assessment is a first step in this process.

Answer: Transconjunctival blepharoplasty and eye shape Thank you for your photo. When your eye bag fat is removed, there is slightly less support holding up the lower eyelid with the decreased volume. This is a common issue with the transconjunctival approach so I modify my technique to remove the eye bag and maintain the support of the lower eyelid. That being said, I do find that most eyelids strengthen up again over the course of a year. I sometimes have patients tape or perform eye blinking exercises in order to speed up the restoration of the lower eyelid position. For the remaining patients who still have an issue, Restylane can be used as a filler to recreate support for the lower eyelid or revision surgery with lid tightening procedures (Canthoplasty or canthopexy) is necessary. Hope this helps! Johnson C. Lee, MD Board-Certified Plastic Surgeon

Answer: Canthal repositioning You are still early in the healing and, as noted below, there are a number of reasons that your eye shape may change post-surgery. That being said if the shape of the eye is not to your liking after having healed one could consider repositioning your canthus with either a canthopexy or canthoplasty. I would continue to follow closely with your surgeon and wait until fully healed before considering any additional procedures.

Answer: Scarring your eye shape often changes after removal of fat and skin. There is no guarantee that this will return to your previous shape, as the reason for the almond eye shape may have been the fat pads (which is now removed). You are however still early in your recovery and I would suggest waiting at least a year for all your tissues to become supple and return to their original shape.
