I had neck lipo and now have excess skin. Do I need a full neck lift or are there other small surgery options? (Photo)

Unfortunately is common to end up with a "turkey neck" after liposuction under the chin.  There are a few reasons for this, but the most common is that fact that the surgeon did not take the time to adequately assess the quality of the facial and neck skin beforehand.  As a general statement, once a person enters his/her 40s, there has been enough laxity of the skin and muscle so that simple under-the-chin liposuction is not a favorable choice.  This can actually make the neck appear worse afterwards.  In your case, based on your picture only, I would recommend a full lower facelift/necklift to tighten up the muscles/skin and give you back your facial shape and profile.  Depending on your surgeon, this procedure can be performed under local anesthesia and oral sedation (as we have done in our office for almost a decade).

You are a great candidate for a neck lift and a face lift.  It appears you have mild laxity under the chin, in the nasolabial folds and the jowls.  All of those areas will be improved with the surgery.  Fractional carbon dioxide laser would enhance your results further, but you would need surgical lifting as well for optimal results.

A Neck Lift can most definitely help your situation. The advantage of a Neck Lift is that neck muscles can be tightened in order to provide more contour to the area, and excess skin is also removed where necessary so you won't have to worry about a turkey gobbler any more!

I would try to avoid a piecemeal approach to your situation.  You already had some liposuction done and ultimately it did not get you what you wanted.  If you went with another smaller procedure, I don't think you would be happy in the end.  It would be best to get at least a neck lift with muscle tightening to address your problem.

 This is why we advise patients to have a facelift at the same time with the neck as well because once the fat is removed if the excess skin is not removed it will sag. Do not make the mistake of doing a "little" procedure  to fix this. You need a neck lift with or without some additional removal of skin in front of the ear.

I treat this everyday in my practice. You may be a surgical candidate but you do not need surgery for improvement. Properly performed liposuction with energy based devices and physical fat removal should do fantastic to remove the fat, lift the skin, and contour the jawline.  It depends on the techniques of the person performing the procedure.  I pretreat everyone with Botox 1-2 weeks before surgery to relax the neck muscles, allowing for better retraction of the skin after surgery. Then after treatment we do skin tightening with Venus Legacy to improve the results and decrease the neck swelling. I suggest you only see an expert for treatment of this area as a meticulous surgeon is required to get great results and complications in this area can be devastating both medically and cosmetically. A new medication that is used to remove chin fat with injections called Kybella may also be an option, as it secondarily allows for skin tightening/lifting as well. Neck liposuction, as a single procedure, is a great procedure that offers tremendous results, but should be reserved for those who are good candidates and have skin that can "retract" or self tighten after the procedure.  That is why liposuction is typically reserved for patients in their twenties and thirties.  It is important to realize, that as we age, the laxity and fullness we see in the neck may not be just excess fatty tissue and/or skin.  There is also a muscle layer that is "hanging down" as well.  The muscle (called the platysma) is often seen as bands or cords but sometimes it is not exposed because a blanket of fatty tissue is covering the muscle.  As a result, liposuction is not enough and a neck lift is required.  A neck lift or face/neck lift will tighten the muscle, sculpt any fatty tissue as needed, and remove the excess skin.
