Hallo, i wrote a question about my hair some time ago. I am 30 years old and take finasteride since 9 years.i have thinning hair on my entire upper head. I want to get an FUE-Hairtransplant with 4000 Grafts. Im a bit worried about my donor area because it is looking thin. I was wondering if maybe scalp-micropigmentation could be the key for me. Maybe a combination of SMP and FUE? Or just SMP? What are you thinking about my donor? I am adding some new Photos. Thank you very much for your help.

Answer: Smp first? I'm happy to hear you are on finasteride for so long. Looking at the pictures you have diffuse loss throughout the top but still a considerable amount of hair. My concern with the amount of hair you still have and the wide pattern of loss is you might not see enough of a huge visual change to justify using so many grafts with a transplant. Remember donor grafts are (currently) limited in number over your lifetime. What I see most in the pics is the color contrast between your dark hair and the lighter skin underneath. I would recommend finding an excellent smp provider and trying that first. If done well, that might get you where you want to be. In the future if an area continues to thin out, try an fue at that point and concentrate it where it will make a big change.

If not done by an experienced and qualified hair transplant surgeon, these 4,000+ graft sessions can increase your risk of shock loss and scarring. Leaving the finished result less than desirable. When such a high number of grafts are needed to achieve the desired result, more than 1 surgery may be needed. Splitting the procedure into 2 greatly reduces the risk of shock loss and scarring.

Answer: FUE with Scalp Micropigmentation Scalp micropigmentation may help you more to solve that problem of hair density . You have to know that 4000 grafts are very big number to transplant that vertex area and even you will have that operation ıt would not cover the thinned area and not provide you a natural density that you wish .

If you have thinning in the donor area, depending upon the degree of thinning, that may be a contraindication for surgery. Also, if you do have thinning in the donor area, you may not be able to obtain 4000 FUE grafts. SMP is a technique to look into if you want to help camouflage your scalp.
