I had a Primary Rhino 4 years ago, and not satisfied with my nose shape. I found a wish pic of a girl that I think I can have a similar look to which is in my opinion a Realistic Goal. Can this be done with Closed rhinoplasty? And what will need to be done to my nose? Will I need alar plasty as well (had it last time too) ?How do I know which doctor will give me this similar result? This is a finesse procedure and don't know who to trust because I don't want another rhinoplasty after this.

Answer: My comments Any major revision rhinoplasty must be necessarily be made via open approach, this to begin with.Second, being the cosmetic judgement of a nose always partially subjective... I can say that your current result is not the best possible and you are candidate for a revision, but... the wish pic is a very bad result, with a pinched deformity, inverted V problem, etc.If we could see the whole set of views for your nose I could provide further opinion, but for sure the model shown as desired result is not a nice and natural nose at all.

Answer: Can a Closed Revision Rhinoplasty make my tip + bridge smaller and more narrow like the model's photo? The results you can achieve are dependent upon your existing anatomy and the expertise of your surgeon. It seems possible you can achieve a narrower nose based upon your picture. After 30 plus years doing rhinoplasty I recommend you find a surgeon who is certified by the American Board of Plastic Surgery.

Answer: Revision rhinoplasty First of all, an in person examination would be required to see if you can get that narrow. Depending on your bone structure, we are limited on how far we can move the nasal bones in without completely obstructing your breathing. Sometimes grafts can be used to create that narrow look but then the profile may be raised a bit. Since there are so many variables, you should see a surgeon who can use computer imaging to show you what they think is possible with your starting point. Best of luck
