My breasts are very saggy, heavy and the nipples point down. What would be the proper way to correct this?

From your description, it sounds like you would be an excellent candidate for breast lift with augmentation.  A lift would reposition your nipples and reshape your breasts, giving them a more youthful appearance.  An augmentation would give you more volume.

To reshape breasts that are sagging, a breast lift will be required. If more volume is needed, a breast augmentation can be performed in conjunction with the breast lift. Good luck!

Some type of breast lift surgery will be appropriate.  Use implants if you are willing to be larger with fullness of your upper pole.  Just a breast lift if you are willing to be smaller.  A breast reduction if you want to be smaller.  All will have same or similar scar and will reduce the diameter of your areola.Best Wishes,Nana Mizuguchi, MD

You currently are a 32D and have ptosis which is
drooping of the breast tissue. The
correct operation to address this would be a mastopexy or a breast lift. I would not recommend placing implants at the
same time. You already have quite a bit
of breast tissue resulting in a D cup and adding an implant to this would just
create heavier breasts and further accentuate your ptosis. When placing an implant and doing a mastopexy
at the same time, the mastopexy has to be conservative so as not to disrupt the
blood supply of the nipple/areola.
However, if you are doing a mastopexy alone then that surgery can be
more aggressive to tighten up the skin. 

Congratulations on your
weightloss! To perk up a drooping breast, your surgeon will remove excess skin,
reposition the nipple, and redrape and tighten the remaining skin to support
the breast. The areola may also be reduced in size. A mastopexy (breast lift)
raises droopy breasts from one to several inches and excess skin is removed. If
you're sagging or asymmetrical, and are looking for perkiness, you'll need a
lift--or, if you also want more volume, a lift and implants.

It sounds like you would benefit from a breast lift combined with an implant. The breast lift pushes the drooping breast upward and the implant provides fullness at the top. Don't expect to have a great deal of fullness in the upper breast since the lift is not powerful enough to do that. Also be careful of too large of an implant since this will lead to a return of the drooping sooner than you would like.Good luck!

Your description is a common one, and the best solution for you is a breast lift - without implants.  Just because you say you lost volume, the breast lift will definitely give you more volume, and after healing, 6-12 months later, if you still want more volume, implants can then be used.
