I had a breast aug 4 years ago and although I get many compliments on them id like better cleavage. My boobs were far apart to begin with and so the doctor said he placed an internal bra when he did the augmentation. However, I had high profile implants 450 CCs and I wonder if I had gotten a lower profile but larger size if theyd look better. Is it possible for me to improve my cleavage while keeping my nipples centered or would they move farther apart if my implants were repositioned closer?

An examination is needed to see if anything can be done. Typically, once the implants and capsules are removed, scar tissue forms and tightens the open area where the implants were.
I would suggest discussing this with your surgeon. Typically, more is added to the expander to give the implant a little more room, unless textured anatomical implants are used.
Although the photo doesn't depict a pronounced case of symmastia, corrective measures are still necessary to establish a safe distance between the implant pockets by suturing the medial space rather than excising tissue. Alternatively, one may opt to replace the implant above the pectoralis...
