Right now I'm quite disappointed with the look of my 550cc overs. Although my skin is thick with good elasticity the implants have moved down quite a bit. Its been 3 years since I had my BA and due to a ruptured implant I'll need to do a revision. This time I'm thinking about downsizing to 450cc. Will going under the muscle help them look perkier? I have uploaded a picture of me and a picture of the result I would like!

However, I would recommend changing position of smaller implants to a dual plane, subpectoral position. In most instances, this also means a breast lift would be indicated. The weight of these implants exceeds the ability of your skin envelope to hold them. As a result, your breasts have descended. In order to create the desired effect, you will need a new force to reverse descent. The technique I recommend is Explant with The Bellesoma Method. This technique will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained. At the same time or later, fat transfer can be performed if more volume is desired. Answer: Revision Downsizing can be helpful to decrease the weight of the implant on your tissue. Your pocket can then be tightened as well. With smaller implants, hopefully you will have less sagging in the future.

It is likely that significant adjustment of the surrounding breast implant capsules will be necessary to improve your outcome.If I were seeing you in consultation I would ask that you communicate your goals with the help of goal photographs (as you have done here) and computer imaging. Terms such as "perkier" or "C or D cup" are subjective and may mean different things to different people. My best advice: seek consultation with a few board-certified plastic surgeons who can demonstrate significant experience achieving the types of outcomes you will be pleased with.
