I have uneven under eye area when I smile - one side forms a much bigger "pad" than the other side, which has caused that side to develop much deeper tear trough wrinkles than the other side. When I'm not smiling both sides look normal and even. I'm concerned that it's just going to get worse as I age. Concerned with hollowness if go under the knife to remove it. I'm also thinking of getting some kind of facial filler (I'm in my early 30s) so not sure how that will affect things.

Answer: Under eye concerns It appears you have some hollowness under the eye. Fillers do a nice job of adding volume and disguising under eye irregularities. I recommend visiting an expert injector in your area for a consultation. Fillers are a great non surgical option and can last up to a year.

Answer: You don't have under eye bags here. You are mostly hollow. However, the world is a scary place. If you ask enough doctors to remove your lower eyelid fat, someone will do it. I don't recommend that. You do have a very heavy upper eyelid and that can be addressed with surgery. The under eye area needs volume. I would recommend the hyaluronic acid filler Restylane for this area. I recommend starting with a detailed personal consultation.
