1st gynecomastia surgery April 2015 lipo and excision. Left crater in left, protrusion still on right side. 14days post revision op, crater on lift fixed. 100ml of fat removed from right, surgeon said he found thick hard scar tissue that he couldn't remove with cannula. Swelling has pretty much gone, leaving my nipple sticking out the same way as before with just less fat around it. The scar is very thick, hard and wide. What are my options? Injections? More surgery? I massage and wear garment.

The vast majority of gynecomastia patients need the benefit of tissue removal through a tiny incision about the areola. There really isn't any justification for not removing the tissue beneath the areola that is the cause of the puffiness. I can see that you have residual tissue behind your areolas.
