Old/thick/hard/large scar tissue build up revealed after gynecomastia revision surgery causing protrusion. Advice? (Photo)

Question

1st gynecomastia surgery April 2015 lipo and excision. Left crater in left, protrusion still on right side. 14days post revision op, crater on lift fixed. 100ml of fat removed from right, surgeon said he found thick hard scar tissue that he couldn't remove with cannula. Swelling has pretty much gone, leaving my nipple sticking out the same way as before with just less fat around it. The scar is very thick, hard and wide. What are my options? Injections? More surgery? I massage and wear garment.

Scar tissue can be tough and sometimes residual breast tissue, scar formation and even hematoma can make it feel tougher on on side versus another. If you are fresh from surgery, it would be reasonable to wait and see how things heal before jumping to surgery again. Your surgeon may recommend scar massage after things settle to help soften the scar. Best, Dr. Ali

Normally with gynecomastia there is a variable amount of dense breast tissue beneath the nipple.  This can be removed through a small periareoloar incision.  You may have an area of fatty necrosis as well that presents as a hard ball beneath the skin.  If this is the case, it will have to be excised.  Talk with you Board Certified plastic surgeon.  Good Luck!
