Thank you for sharing your question and photographs.  Though you may have residual swelling that is masking your long-term results, if you feel firm tissue behind your nipple and areola this is most likely residual gland that will need to be surgically excised in order to achieve a flat appearance.  Only through an in-person examination can this be determined.  Voice your concerns to your surgeon, they should be able to offer you the best advice.

The vast majority of gynecomastia patients need the benefit of tissue removal through a tiny incision about the areola. There really isn't any justification for not removing the tissue beneath the areola that is the cause of the puffiness. I can see that you have residual tissue behind your areolas.

It can take anywhere from 3-6 months for the swelling to fully resolve so this is, hopefully, swelling. I would suggest that you wait until you are 6 months post op. If the issue hasn't resolved then it is glandular tissue that will need to be surgically excised. Best of luck.
