I am undergoing rhinoplasty in January 2019 to have the bridge and tip of my nose reduced and also have my columella straightened and slighted lifted. However my nose is considered to be wide and my skin at the tip of my nose is thin. I have seen multiple consultants and one told me that if I went ahead with this, I would be left with a “flat looking nose” at the front. This really concerns me because I’m quite happy with my front view I just want to fix my profile and oblique view.

I recommend you seek re-evaluation with your surgeon to make sure you and your surgeon are on the same page for goals and expectations of surgery. Reducing the dorsum height may make the dorsum appear more flat on frontal view. An experienced rhinoplasty surgeon will be able to find the right balance of taking down the dorsum enough while preserving the refined appearance of the dorsum or figure out with the patient which goal is a higher priority. If your nose is fairly wide on frontal view, there are some techniques such as bringing in the nasal bones that can help reduce the width as well.

Answer: Rhinoplasty Even with this many photos it is very hard to tell you what can be done without in person evaluation. Some changes in tip and dorsum could be considered but without good analysis and discussion about your expectation I could not suggest more accurate plan of treatment. Good luck.

You don't have an excessively large nose now, but to achieve your most elegant look certain steps must be taken. The profile can be softened, the tip can be refined-all through the closed technique. This technique gives the best opportunity - in my opinion- for the best result. Frontal width can be adjusted as needed. In the right hands you should not have a "flat looking nose".
