Thank you for your question.It is difficult to say what this "bleeding" is without any photographs. It is most likely some serous fluid mixed with old blood coming out from between the sutures. Bleeding at 4 weeks post op should be examined by your surgeon. If it is small areas where perhaps a small scab has come loose then there is typically no cause for concern. As it's better to err on the side of caution, I recommend speaking to your surgeon about this situation.

The best course of action would be to schedule an appointment with your board certified plastic surgeon as he knows your condition best. It is uncommon for 4 weeks post op to be bleeding.
It's hard to say if your bleeding is normal without additional details and photos. Small, pinpoint bleeding may occur if there had previously been scab or a spitting suture. A large amount of bleeding would be unusual this far post op. I recommend seeing your surgeon to assess the situation and determine if any intervention is necessary.
At four weeks you should not be bleeding and should not be too swollen. Perhaps you have a hematoma or old blood clot that is trying to drain. I would suggest going back to your surgeon and being checked.

You may want to have your PCP run some blood work to make sure that all of your micronutrients (vitamins and minerals) are in normal range. Make sure that you have adequate hydration. You may incorporate gentle stretching exercises that can help with this as well.
It is best to have your surgeon examine the area to determine the best course of action. It is unlikely the surgery will be canceled unless there is infection present or the rash/swelling is excessive. Your surgeon will advise you.
