When the contour of the neck is smooth, and the definition is lacking, the problem comes from volume rather than skin.With an access incision under the chin it is possible to address this volume significantly and the skin tension would remain unchanged. This is referred to as anterior neck lift or short scar neck lift. Some surgeons have suggested that the procedure can be carried out under local anaesthetic with intravenous sedation, but most surgeons would advocate a general anaesthetic. I have not heard of anybody saying they have carried out this type of surgery purely under local anaesthetic.

Depending on the amount of improvement needed/desired, incisions for a necklift can be placed behind the ears and/or in the submental neck area, just under the chin. Generally, incisions placed in front of the ears result in a face lift. Whether or not this may be performed under local anesthetic also depends on how extensive of a neck lift is being performed. I recommend scheduling a consultation with a Board Certified Plastic Surgeon to obtain a professional recommendation and treatment plan.
In selected candidates who do not have much of skin redundancy it is possible to achieve all necessary improvement in the neck and jaw area through a sub-mental (bellow chin) incision; no need for skin excision in front and behind ears. However, without in person evaluation it is impossible to tell whether you are a candidate for such a procedure. Good luck.

In my practice, I perform this with either general anesthesia or conscious sedation, both of which are administered by a MD board-certified anesthesiologist in my practice. If this is being offered using only local anesthesia (without a board-certified MD anesthesiologist), this generally involves only skin tightening and not muscle tightening and will not provide a long lasting result. This decision shouldbe based on a detailed discussion with equal input from both youand your surgeon. Make sure you specifically lookat before and after pictures of real patients who have had this surgeryperformed by your surgeon and evaluate their results. Hi LondonGB,An isolated neck lift, not addressing the jowls or lower face, can absolutely be performed with incisions that start at the earlobe attachment and proceed posteriorly behind the ear and a bit into the hairline. That means that almost all of the incision is hidden behind the ear. You will have one small incision under the chin that hides nicely, usually in a crease if you have one.And we absolutely could do this under local anesthesia with some light oral sedation.Please note that this is my preference, but not necessarily that of other surgeons.
