Thank you for your question. Skin discoloration is a very prevalent problem in women over the age of twenty. It is also increasingly common in men with increased sun exposure. Risk factors include thyroid disease, sun exposure, hormones, pregnancy, family history, PIH, and birth control. Melarase AM and Melarase PM have been extremely effective in reducing pigmentation associated with skin discoloration and hyperstimulation of melanocytes. In my practice, patients with hyperpigmentation benefit from twice daily application of the creams. Melarase AM in the morning, followed by Melarase PM in the evening. Melarase AM contains four active sunblocks, so sunscreen is not needed unless your sun exposure is very high or you had a recent procedure/laser. SPF of 30. Melarase PM contains active skin lighteners to even out skin pigmentation. If you are concerned about the development of pigmentation, contact our office to discuss details and to obtain a regimen for Melarase. I find that low energy lasers with microneedling/PRP do very well for melasma. fractional ruby laser which we have does great as well and is combined with other treatments. low strength peels can give great outcomes as well with skin care. Please see an expert for an evaluation. Best, Dr. Emer.
Melasma is a common skin condition of adults in which light to dark brown or grayish pigmentation develops, mainly on the face. Although it can affect both genders and any race, it is more common in women and people with darker skin-types who live in sunny climates. PRP has proven effective in the treatment of Melasma. The one good thing about PRP for Melasma is the fact that PRP won’t make the condition worse unlike IPL, Fraxel or other treatments. PRP injections work by supplying growth factors to reduce the pigmentation. And being an independent treatment with no downtime, it can be done in conjunction with conventional treatments for Melasma to add and enhance the effects.Seek out a doctor who is experienced administering PRP treatments to see if it is the right choice for you.
At my practice we offer retinol peels, a possible safe, alternative option for you, based on your skin type. We have seen incredible results with reduction of melasma and other hyperpigmentation, particularly in our patients with darker complexions. This medium depth peel is painless with virtually no downtime aside from 3-4 days of peeling skin. Melasma can be difficult to treat, particularly when the patient is darker complected but there are certainly options available that will work for you.
Though reported to help in certain cases of melasma, it can worsen other cases, especially if a tyrosinase inhibitor is not used with this treatment. Melasma can not be completely cured, however effective treatment are possible. This is because your skin is extremely sensitive to UV and even the smallest amount can stimulate your pigment cells to produce colour. The mainstay of treatment is strict UV protection- hats, sunglasses and SPF every 4 hours. I prefer to use a combination of creams and laser for melasma. Hydroquinone can be used (5-8%), along with vitamin A creams. I usually start my patients on laser (low dose Q switch or Picosure) a few weeks after they commence on creams. Another laser I use is the Clear and Brilliant in the Permea setting combined with creams. In some cases I combine glycolic AHA peels, and in other cases I use a tablet called Tranexamic acid to help. Melasma treatments should be tailored to each patient. All the best, Dr Davin Lim.
