A lower blepharoplasty uses transposed fat to fill out the hollows of the eyes. However, you would get more improvement by a rhinoplasty. Your nose has shifted to the side and a rhinoplasty with possible septoplasty would give much more improvement of your face than a lower blepharoplasty. Best Wishes,Gary Horndeski, M.D.
Beware though that if you ask, some will operate on you whether you need surgery or not. As I look at these photographs, I do not believe that your prior filler has be adequately removed. Your lower eyelids exhibit a type of lumpiness associated with retained filler. Also when I look at your face overall, I do wonder if you have significant retained filler in your cheeks. The material overtime does not really disappear. It drifts in the face and as it ages, it holds more and more volume. For some reason, we tend to accept these changes and assume they are due to aging rather than due to aging filler. If I were to see you, I would ask you about how much filler you have had in the past. Likely enough to account for the changes in your face. By removing this old filler, you are likely to look 10 to 15 years younger. There is no substitute for a detailed in person assessment. Hope this helps.
Based on these photos, it appears that a combination of lower, transconjunctival blepharoplasty and TCA chemical peel would be a good procedure for you. In person consultation with a board certified plastic surgeon with good reputation in periorbital rejuvenation is recommended. Good luck.

You may benefit from a brow lift done in the hairline. It will decrease the vertical distance of the forehead, lower the hairline and pull up the upper eyebrows. Your eyebrows are slightly below the orbital rim and they should be at the level of the orbital rim. Best Wishes, Gary Horndeski, M.D.

Although skin care is important, it only helps with skin, not the fat pads or contours underneath. This photo is out of focus and it is hard to tell your anatomy. Younger patient with lower eyelid issues typically have a negative vector. This means the fat is pushing forward more and creating a...
