Many patients do not want a lift or try to get by with just a donut lift. In your case, you definitely need a lollipop mastopexy and definitely will not "get away" with a donut lift. The reality is that is your anatomy. The good news is you should be able to have an excellent result with a talented surgeon. Best wishes to you.

You need the lift. measurements are needed to see if the doughnut lift or the vertical lift if needed. If comfortable, let your surgeon do the lift since he will not charge you.

I would recommend implant exchange, partial capsulectomy because your implants are too high and are pushing the nipples down, contributing to your sagging, along with a breast lift to tighten the excess skin.
