Subcision, followed by Sculptra, Microneedling for Rolling and or Boxcar scars - what is the best treatment option? (Photos)

I’ve already had a moderate intensity C02 and Microneedling RF x3 done (probably spaced out too far apart). I was told by a non-expert that it would’ve been more beneficial to get subcision done first, then proceed to the other treatments, getting some type of filler in between to keep the scars from re-anchoring or retethering. I also would like to know if my scars are prinarily box Car or rolling scars, as it’s very difficult for me to tell the difference.I’d like to know if the treatment recommendations for the two types differ? Please help! Thanks. I also wouldn’t mind treating smile lines while at it..…

I am not surprised by the lack of success with your prior device treatments, since I have been cautioning my regular followers for years that the hard science to back up the use of these expensive gizmo treatments lags well behind the heavy marketing hype and social media buzz to promote them. Sorry.
At least from the photos provided the right cheek appears to have a number of wide-based rolling scars. Subcision, which I have been performing for over thirty years, has a proven track record of safety and efficacy for elevating and smoothing both rolling and boxcar scars and would indeed be an appropriate in this case in the hands of an experienced, board certified cosmetic dermatologist scar treatment expert. Given the number of larger rolling scars, a Field Subcision of the right cheek might be in order before addressing any direct subcision of any individual scars. Subcision can be performed with/without the addition of a filler or biostimulator. The nature and extent of the scarring on the left cheek are unfortunately harder to discern from the photo provided. However, if there are ice pick scars, these can be treated using TCA CROSS.
