I was wondering if you could explain to me exactly what it means when the surgeon says that I will not have enough bone support to extend the chin forward enough to where it would be more prominent than with my chin implant. Would it be worth it to get a second opinion from another oral surgeon?Thank you for your time, Julia

You will need to see an Oral/Maxillofacial surgeon to see if othognathic surgery, genioplasty or using an implant is best for your specific case. Answer: Sliding Genioplasty It would be very uncommon that the chin bone could not be advanced or provide as much projection as that of a chin implant. I would recommend a second opinion. It would be helpful to see pictures of your profile as well as a cephalometric x-ray to determine your bone thickness.

A sliding genioplasty is performed by an oral surgeon under general anesthesia in a hospital with an overnight stay, since the mandible is being cut, slid forward, and plated into the new forward projection. Answer: Chin surgery Hi Julia, If your surgeon says this then most likely they are correct. You can not advance the chin too much. The bone healing would not occur properly. If you need your chin advanced so much you may need jaw surgery instead to begin with.

Answer: Genioplasty There is a way to actually 'see' what the surgeon means; the cephalometric xray/side view xray does show the 'thickness' of the chin bone area. Therefore you can calculate the 'amount' of advancement that is possible. However, even if the amount is'less than what you want', the surgeon can add 'implant strips' in front of the advancing bone; so you get a fuller effect. Quite common for some of us when approaching large advancements. The genioplasty is a better option to control the position and height of the chin and the 'stretching' of the muscles under the chin to give a flatter look
