Hello, My nose broke and it healed crooked, I would like to know if a rhinoplasty could straighten my nose and if I will need grafts to straighten the bridge. If yes, which kind and is it a unilateral spreader graft on the concave side ? Thank you
Answer: Would you consider a permanent non-surgical nose job? If you don't have any trouble breathing through your nose, and if your goal is to just repair the indentation along your right nasal sidewall, depending on your examination, microdroplet Silikon-1000 injections may be considered for permanently achieving a straighter nasal appearance. Thank you for your question. Sincerely, Dr Joseph

Answer: Post-traumatic rhinoplasty. hank you for your question. Using a graft to create the impression of a straighter bridge by building on the side that appears more sunken in is also a way to achieve a wider nose. If this is not part of your design, the rhinoplasty requires resetting the bridge to improve the direction. In general, this may also require grafts, and most surgeons prefer to use grafts to support the nasal shape that is created through rhinoplasty. If you would like to explore the appearance of the nose by just adding some volume on the concave side, you can use injectable fillers. This will give you a good understanding of what a unilateral spreader graft could do for you. However, I suspect it is not the answer you would be looking for.

Answer: Straightening a crooked nose Hello and thank you for your question. Based on your photograph, you may benefit from bilateral spreader grafts, tip refinement, and other maneuvers to help straighten your mid nasal vault. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: Straightening a broken nose Hello,I wish it was as simple as defining a problem with the nose and applying a specific technique to fix it. That approach would make rhinoplasty an easier to surgery. In realty, rhinoplasty is more like a puzzle that the surgeon has to solve during the surgery. There are many different techniques and they can all be performed with slightly different modifications. There are also many different variations to noses; their intrinsic anatomy and the resulting anatomy after injury. So we really have to do a thorough exam to understand why it's deviated in order to have a better idea of our plan. ... and that plan may change during surgery. This is why it's like a puzzle.... good rhinoplasty surgeons are able to perform many different techniques in different ways and different combinations to get the best result. A unilateral spreader graft on the concave side is a decent technique for camaflouge. As a stand alone technique, it doesn't treat the deviation, it disguises it. This will make the nose wider in that area. Such can also be accomplished with filler. Using spreader grafts to reinforce bent areas, or in combination with clocking sutures can help treat the deviation. We can go on for chapters discussing the deviated nose, but the point is that you should consult with at least a few surgeons that specialize in rhinoplasty and choose one that you trust will make the best decisions for your needs during surgery. Good luck!
